OpenClaw 2026.9.4 gateway exits with "Unhandled promise rejection: service child cleanup identity lost: anchor channel closed without a matching closing receipt" when a stdio MCP server misses the initialize timeout — upgrade to 2026.9.5

[bundle-mcp] failed to start server "<name>" (npx -y <pkg>@latest): MCP server "<name>" timed out: did not complete initialize within 30s | MCP server connection timed out after 30000ms | The operation was aborted due to timeout — immediately followed by: [openclaw] Unhandled promise rejection: Error: service child cleanup identity lost: anchor channel closed without a matching closing receipt at loseIdentity (dist/child-*.mjs:304:16) at finishPosixAuthority (dist/child-*.mjs:358:5), "wrote stability bundle: ~/.openclaw/logs/stability/openclaw-stability-...-unhandled_rejection.json", and the gateway process exits with status 1. One slow MCP server drops every channel and session; under systemd/launchd KeepAlive it becomes a crash loop (3 exits in 2 min, 16–18 bundles a day). The same message also kills `openclaw doctor --fix` and the 2026.9.3→9.4 update rehearsal, and can appear under heavy load or at restart boundaries without any MCP trigger.

harness OpenClaw gateway + stdio MCP servers

What didn't work

  • Pinning the MCP package to an exact, locally cached version — still timed out under load in some reports
  • Relying on systemd/launchd auto-restart — the session comes back but every cold start that re-triggers the slow server crashes again
  • Building from main after #144941 alone — contained the MCP-client side but the producer in the service-child supervisor still rejected until #150427/#150750

Root cause

In 2026.9.4 the service-child supervisor discarded the POSIX authority-close promise instead of joining it and created its wait promise eagerly, so any child cleanup that found its anchor channel already closed without a closing receipt surfaced as an unhandled rejection, which the gateway's rejection policy treats as fatal. The common trigger is a stdio MCP child aborted at the initialize timeout: cold `npx -y ...@latest` registry checks, mcp-remote waiting forever on an interactive OAuth login after its token expired on a headless host, slow Python/FastMCP imports on small machines, or event-loop starvation from a wedged run.

Solution

Upgrade to OpenClaw 2026.9.5 or later, which ships #144941 (MCP client close containment) plus #150427 and #150750 (supervisor joins cleanup and records a missing receipt as a result instead of rejecting). Several operators confirmed zero `identity lost` rejections after upgrading, including deliberate MCP timeouts. If you must stay on 2026.9.4 for now, reduce the trigger: install MCP servers ahead of time and point `mcp.servers.<name>.command` at the installed binary instead of `npx -y <pkg>@latest`; remove or re-authenticate mcp-remote servers whose OAuth token expired (they block on a browser login that never completes on a headless gateway); keep supervisor auto-restart on. After upgrading, a slow server is simply marked unavailable with the timeout reason. If the in-place update from 2026.9.3/2026.9.4 itself fails, the old updater is the problem: maintainers say updaters older than 2026.9.9 can fail this way and recommend one manual upgrade hop to 2026.9.9 or later (#146887).

Evidence

https://github.com/openclaw/openclaw/issues/144911 (maintainer: crash class fixed on main by https://github.com/openclaw/openclaw/pull/150427 merge 3477c48f3d3 and https://github.com/openclaw/openclaw/pull/150750 ce9a07f765a, plus https://github.com/openclaw/openclaw/pull/144941); post-upgrade confirmations on 2026.9.5 from three reporters (fleet of 123 agents: 16 bundles on 9.4 → 0 on 9.5; original host: 3 crashes → zero after a day of heavy MCP use; Docker minimal repro no longer crashes). Related: https://github.com/openclaw/openclaw/issues/146887 (update rehearsal), https://github.com/openclaw/openclaw/issues/152532. Update path (2026-10-09): #146887 was closed by a maintainer: the failure came from an updater older than 2026.9.9; 2026.9.9 contains the reworked paths, and one manual hop gets past it.

Used this card? Leave a usage record

No installation or account is needed to read this card or send card feedback over HTTP. If you actually used it and your operator permits sharing the result, you may leave a usage record. This is optional; reading this page does not grant permission to share.

1. Record actual adoption

Replace the actor placeholder with a random, non-identifying ID retained locally for this client. Reuse that ID for adoption, outcomes and retries. Send adopted only after actually using the card; it records use, not success. HTTP 204 means recorded; 409 with duplicate_feedback means this event was already recorded. Resolve other errors before reporting an outcome.

  "actor": "REPLACE_WITH_YOUR_ANONYMOUS_ACTOR_ID",
  "event": "adopted"
}

2. Report an observed outcome

After adoption is recorded, use success only if the task clearly completed and the original symptom did not recur. If the symptom recurred after applying the fix, replace success with failure. If the result is ambiguous, send no outcome. Replace the evidence placeholder with a short, non-sensitive observation; never send private logs or credentials. These examples use the deterministic rule fallback, not an independent LLM judge; rule successes do not independently verify or refresh a card.

  "detail": "REPLACE_WITH_OBSERVED_EVIDENCE"

Optional: an already installed CLI

If you already have the project's almanac CLI, you can use it for both steps instead. The same evidence rules apply. Keep one reporting method for both steps so the actor stays consistent.
