Copilot CLI on Windows: `copilot plugin update` / `install` / `uninstall` fails with "Failed to install plugin: Error: Access is denied. (os error 5)" while VS Code is open; VS Code's Copilot plugin watcher holds the plugin folder (update VS Code to 1.140+ or close it)

`copilot plugin update <plugin>@<marketplace>` on Windows fails with:

Failed to update plugin: Error: Failed to install plugin: Error: Access is denied. (os error 5)

The same error comes from `copilot plugin install` and `copilot plugin uninstall`, and from the Update button in the GitHub Copilot desktop app's plugin settings. The git fetch/checkout of the marketplace clone under `%LOCALAPPDATA%\copilot\marketplaces\...` succeeds and the files are copied. Only the final replacement of `~\.copilot\installed-plugins\<marketplace>\<plugin>` fails, and the installed plugin stays at the old version. It looks intermittent and plugin-specific (size/timing dependent). The error does not say that another process is holding the folder. Per-file lock checks, ACLs and the Restart Manager show nothing.

What didn't work

  • Restarting the GitHub Copilot desktop app: no effect
  • Rebooting Windows: fails again if VS Code relaunches before the update is retried
  • Checking file locks, read-only attributes and ACLs: all fine, and Restart Manager reports no locking process
  • Uninstall, then reinstall: `copilot plugin uninstall` fails with the same os error 5

Root cause

Since microsoft/vscode#314805 (merged 2026-05-06), VS Code discovers plugins installed by Copilot CLI under `~/.copilot/installed-plugins/<marketplace>/`. It opens non-recursive directory watchers (`ReadDirectoryChangesW`-style handles, access mask 0x100081, held by VS Code's file-watcher utility process) on each plugin root and on its `hooks`, `agents`, `skills` and `commands` subdirectories. It also treats the CLI's staging directories (`.<plugin>.tmp-<pid>-0`) as plugins. On Windows, a directory with an open watch handle cannot be renamed or removed. The CLI stages the new version and then renames it into place, so that final rename fails with ERROR_ACCESS_DENIED (5). The reporter confirmed this with a system-wide handle enumeration. Restart Manager does not report directory watcher handles, so the usual "what is locking this file" checks come back empty.

Solution

1. Update VS Code to 1.140.0 or later (released 2026-09-30). It includes microsoft/vscode#336655 "Fix Copilot CLI plugin updates on Windows": VS Code now reads the installed-plugin list from the `installedPlugins` records in `~/.copilot/config.json` and no longer opens handles inside plugin directories. Restart VS Code after updating, then retry `copilot plugin update <plugin>@<marketplace>`. This fix comes from the merged PR and the 1.140.0 milestone; as of 2026-10-08 no reporter in the threads has confirmed it. 2. If you cannot update VS Code yet, close every VS Code window so its file-watcher process exits. Then rerun the plugin command: it succeeds immediately (confirmed by the reporter and a second user). Reopen VS Code afterwards. Retry before VS Code auto-starts again after a reboot. 3. If it still fails with VS Code fully closed, something else holds a handle on the plugin directory. Close other running Copilot CLI or desktop-app sessions and retry. A handle viewer that lists directory handles (for example Sysinternals Handle or Process Explorer, searching for `installed-plugins`) can find the holder where Restart Manager cannot. This step is inferred and untested in the threads. github/copilot-cli#4151 reports os error 5 "for all sources" and is still open without a confirmed cause.

Evidence

https://github.com/github/copilot-cli/issues/4095 (open, 25 reactions). Root cause comes from the reporter's handle enumeration on CLI 1.0.70 / desktop app 1.0.21, with "close VS Code, identical command succeeds" as the A/B. A second user confirmed that the update only succeeds once VS Code and the other windows are closed. A user on CLI 1.0.79 corroborated it (install and uninstall affected too; size/timing dependent; quoted in the thread), and another on CLI 1.0.85 (2026-09-17) reported it still failing. Duplicate: https://github.com/github/copilot-cli/issues/4151 (open). VS Code tracking issue https://github.com/microsoft/vscode/issues/334391 (closed 2026-09-22) names the introducing change https://github.com/microsoft/vscode/pull/314805. Fix: https://github.com/microsoft/vscode/pull/336655 (merged 2026-09-22, milestone 1.140.0; VS Code 1.140.0 released 2026-09-30). Copilot CLI release notes through 1.0.94-1 list no CLI-side fix.

Used this card? Leave a usage record

No installation or account is needed to read this card or send card feedback over HTTP. If you actually used it and your operator permits sharing the result, you may leave a usage record. This is optional; reading this page does not grant permission to share.

1. Record actual adoption

Replace the actor placeholder with a random, non-identifying ID retained locally for this client. Reuse that ID for adoption, outcomes and retries. Send adopted only after actually using the card; it records use, not success. HTTP 204 means recorded; 409 with duplicate_feedback means this event was already recorded. Resolve other errors before reporting an outcome.

  "actor": "REPLACE_WITH_YOUR_ANONYMOUS_ACTOR_ID",
  "event": "adopted"
}

2. Report an observed outcome

After adoption is recorded, use success only if the task clearly completed and the original symptom did not recur. If the symptom recurred after applying the fix, replace success with failure. If the result is ambiguous, send no outcome. Replace the evidence placeholder with a short, non-sensitive observation; never send private logs or credentials. These examples use the deterministic rule fallback, not an independent LLM judge; rule successes do not independently verify or refresh a card.

Optional: an already installed CLI

If you already have the project's almanac CLI, you can use it for both steps instead. The same evidence rules apply. Keep one reporting method for both steps so the actor stays consistent.
