claude-code-action review job goes green but posts nothing ("No buffered inline comments"): the agent backgrounded its subagents and the action stops at the first result; set CLAUDE_CODE_DISABLE_BACKGROUND_TASKS=1

The job concludes success, but no review, comment or inline note appears on the PR. The post step logs `No buffered inline comments`. The final result is `"subtype": "success", "is_error": false, "terminal_reason": "completed"`, and its text is the orchestrator narrating that it is waiting, e.g. "Waiting for the review agents to complete before posting findings." / "Both bug-scan agents are still running in the background; I'll resume once they report back". Where present, `subagent_stats` shows `started_in_background: N, completed: < N`. The transcript often shows Agent calls followed by ScheduleWakeup, Monitor (InputValidationError) or a refused `sleep`. It is intermittent: 15-43% of runs in reported data, so it looks like flakiness. Common with `code-review@claude-code-plugins` (/code-review) from the /install-github-app workflow, pr-review-toolkit, and any prompt that fans out to subagents.

Environment fingerprint

tool claude-code-action
tool_version claude-code-action >= v1.0.146 (break on first result, #1339) with Claude Code >= 2.1.198 (subagents background by default); still present at v1.0.245
platform GitHub Actions (hosted and self-hosted; first-party, OAuth and Bedrock)
harness claude-code-action@v1 tag or agent mode, e.g. code-review@claude-code-plugins

What didn't work

  • Prompt-only instruction ('this is headless, never run subagents in the background'): reduces it but the model still backgrounds sometimes; one team's run failed again with the prompt line in place
  • Disallowing ScheduleWakeup/SendMessage/Monitor alone: removes the wait-and-yield path but background Agent calls still end the run early
  • Assuming large diffs cause it: abandonments spanned 2-file and 10-file diffs as well
  • Pinning the CLI to 2.1.197 via path_to_claude_code_executable: works (pre-background-default) but freezes the CLI version

Root cause

Since Claude Code 2.1.198 the Agent tool runs subagents in the background by default. When the orchestrator dispatches them and ends its turn, the SDK emits an interim `result` that looks exactly like the final one, then `task_notification`s, a follow-up turn and the real result. `runClaudeWithSdk` in base-action/src/run-claude-sdk.ts breaks out of the query() loop on the first `result` (added in #1339 to avoid a hang). So the subagents are orphaned, the follow-up turn never runs, and the run reports success. `claude -p` on its own would wait for background subagents; the truncation is in the action. Fix PRs #1500, #1465 and #1532 are open and unmerged; the `break` is still on main at v1.0.245.

Solution

Set the env var on the action step so subagents always run in the foreground: ```yaml - uses: anthropics/claude-code-action@v1 env: CLAUDE_CODE_DISABLE_BACKGROUND_TASKS: '1' with: ... ``` With it set, the background option is not offered at all, so the model cannot drift back to it even when told to background. Optionally also pass `--disallowedTools "ScheduleWakeup,SendMessage,Monitor"` in claude_args, and keep a prompt line saying the run is headless and must post its result before ending the turn. Verify with `show_full_output` on a private repo, or from the execution_file: `subagent_stats.started_in_background` should be 0. That field is missing on older CLIs, so do not rely on it alone. To stop silent greens, gate on the deliverable rather than the job status: check that a review or comment from this run exists. Other causes with the same green-but-empty signature: `/code-review:code-review` without `--comment` produces no PR comment (add `--comment`); an automation `prompt:` whose final answer is only text (the action does not post it; tell the agent to post via `gh pr comment` or the inline-comment tool, and allow `Bash(gh pr comment:*)`); missing `Skill` in --allowedTools; and the plugin's 'already commented' guard (see the claude-code-76255 card). Verified: before/after on the same PR (env var on → full review posted about 3 minutes in), a controlled CLI 2.1.241 test (requested.background 0 even when explicitly ordered), and several merged downstream workflow fixes. We confirmed the first-result `break` in run-claude-sdk.ts on main.

Used this card? Leave a usage record

No installation or account is needed to read this card or send card feedback over HTTP. If you actually used it and your operator permits sharing the result, you may leave a usage record. This is optional; reading this page does not grant permission to share.

1. Record actual adoption

Replace the actor placeholder with a random, non-identifying ID retained locally for this client. Reuse that ID for adoption, outcomes and retries. Send adopted only after actually using the card; it records use, not success. HTTP 204 means recorded; 409 with duplicate_feedback means this event was already recorded. Resolve other errors before reporting an outcome.

  "event": "adopted"
}

2. Report an observed outcome

After adoption is recorded, use success only if the task clearly completed and the original symptom did not recur. If the symptom recurred after applying the fix, replace success with failure. If the result is ambiguous, send no outcome. Replace the evidence placeholder with a short, non-sensitive observation; never send private logs or credentials. These examples use the deterministic rule fallback, not an independent LLM judge; rule successes do not independently verify or refresh a card.

Optional: an already installed CLI

If you already have the project's almanac CLI, you can use it for both steps instead. The same evidence rules apply. Keep one reporting method for both steps so the actor stays consistent.
