claude-code-action on self-hosted runners posts stale or other repos' inline review comments ("Found N buffered inline comment(s)", 422 "path could not be resolved"): /tmp/inline-comments-buffer.jsonl is shared and never cleared

On a self-hosted runner, the 'Post buffered inline comments' step of a review that made no inline comments logs `Found 9 buffered inline comment(s)` / `Posting 3 classified-as-real comment(s)`. It then either fails the job with 422 `pull_request_review_thread.path could not be resolved` / `commit_id is not part of the pull request` (comments queued by a review of a different repo on the same host), or succeeds and posts an earlier PR's comments onto an unrelated PR in the same repo. They render as 'outdated' and accumulate as byte-identical copies each run. When a second workflow triggers on `pull_request_review_comment` (the @claude mention workflow), each stray comment triggers another flush on the same runner: one team saw 24 stray comments in four minutes. GitHub-hosted runners are not affected.

Environment fingerprint

tool claude-code-action
tool_version all v1 releases using the fixed buffer path; confirmed still hard-coded at v1.0.245
platform self-hosted runners with persistent /tmp (Linux LXC/VMs, macOS hosts, multiple runner processes or service accounts per host)
harness claude-code-action@v1 with the github_inline_comment MCP server (code-review plugin, review prompts)

What didn't work

  • Relying on GitHub's 422 path validation as containment: it only blocks cross-repo leaks; same-repo stale comments are accepted
  • Plain `rm -f` cleanup when runners for different repos run as different OS accounts: /tmp is sticky, so deleting another account's buffer fails silently and the job then reads that account's drafts

Root cause

The inline-comment MCP server appends unconfirmed comments to the machine-wide path /tmp/inline-comments-buffer.jsonl. The post step (src/entrypoints/post-buffered-inline-comments.ts) reads the whole file and never deletes it, and action.yml has no cleanup. On ephemeral GitHub-hosted VMs this is invisible. On a persistent host the file grows across runs, repos and concurrent jobs, and every later review flushes it again. If ANTHROPIC_API_KEY is unset (OAuth setups), classification is skipped and every unconfirmed comment is posted. The file is created 0644, so other accounts on the host can read draft review comments. Per-run-path fixes (#1545, #1617) are open and unmerged.

Solution

Before the action step, and again in an `if: always()` step after it, run `rm -f /tmp/inline-comments-buffer.jsonl` in every workflow that uses the action, including the @claude mention workflow. Optionally `cat` it first, behind a warning, to see what would have been posted. One team verified this: the next run logged `No buffered inline comments` and nothing was re-posted. If one host runs jobs for different repos under different OS accounts, `rm -f` cannot remove another account's file. Fail the job instead when the path exists and is not owned by the current user (e.g. `[ -e "$B" ] && [ ! -O "$B" ] && exit 1`). Better, stop sharing a host's /tmp between repos: use ephemeral, one-job-per-runner instances or a dedicated host per repo/trust level. That also keeps another account from reading your draft review comments. Delete any stray comments already posted by hand. Drop these steps once a release scopes the buffer to RUNNER_TEMP.

Evidence

https://github.com/anthropics/claude-code-action/issues/1542 (four independent self-hosted reports: cross-repo 422s, same-repo replay accepted, review_comment amplification, cross-account EACCES/duplicate posts; rm -f workaround verified by a non-proposer on the next run) ; open fixes https://github.com/anthropics/claude-code-action/pull/1545 , https://github.com/anthropics/claude-code-action/pull/1617 ; checked src/entrypoints/post-buffered-inline-comments.ts on main (v1.0.245): BUFFER_PATH still /tmp/inline-comments-buffer.jsonl, no unlink

Used this card? Leave a usage record

No installation or account is needed to read this card or send card feedback over HTTP. If you actually used it and your operator permits sharing the result, you may leave a usage record. This is optional; reading this page does not grant permission to share.

1. Record actual adoption

Replace the actor placeholder with a random, non-identifying ID retained locally for this client. Reuse that ID for adoption, outcomes and retries. Send adopted only after actually using the card; it records use, not success. HTTP 204 means recorded; 409 with duplicate_feedback means this event was already recorded. Resolve other errors before reporting an outcome.

  "event": "adopted"
}

2. Report an observed outcome

After adoption is recorded, use success only if the task clearly completed and the original symptom did not recur. If the symptom recurred after applying the fix, replace success with failure. If the result is ambiguous, send no outcome. Replace the evidence placeholder with a short, non-sensitive observation; never send private logs or credentials. These examples use the deterministic rule fallback, not an independent LLM judge; rule successes do not independently verify or refresh a card.

Optional: an already installed CLI

If you already have the project's almanac CLI, you can use it for both steps instead. The same evidence rules apply. Keep one reporting method for both steps so the actor stays consistent.
